Transcript Request Page

1. Graduates or former students of Academy of Therapeutic Esthetics And Massage or Healing Arts Center may request a transcript at any time following their graduation or separation from the ATEAM or HAC.
2. Transcripts will not be issued until all coursework and required documentation for graduation have been completed, or the student has terminated enrollment in the program.

Transcript requests must be submitted to ATEAM administration in person, by mail, or by email (via the form above) from the email registered to the student while they attended ATEAM or Healing Arts Center. If the student no longer has access to their email address, ATEAM may request other information sufficient to establish that the request is coming from the former student/graduate. Any requests for transcripts by mail must be notarized.
2. Graduates or former students will be required to provide the following information for transcript requests:
   1. Full name (as it appears on academic records).
   2. Date of birth (for identity verification purposes).
   3. Contact information.
   4. Number of copies requested
   5. Recipient details

One electronic copy of a transcript is provided at no charge to former students or graduates of ATEAM. Electronic copies of transcripts for former Healing Arts Center (HAC) students or for electronic copies sent to third parties (e.g., State Agencies, Testing Authorities, other Institutions, etc.) are $10 per copy. Paper copies of transcripts will be provided for $11 per copy to cover the cost of printing and mailing.

Transcript requests may require up to 5 business days for processing from the date of receipt of the request. Transcripts for students who graduated more than 5 years ago from Healing Arts Center (HAC) may require additional processing time.

Transcripts may be delivered via postal mail, electronically (when available), or in person, depending on the student's or recipient's preference.

The Academy of Therapeutic Esthetics And Massage is committed to maintaining accurate and secure academic records. Any discrepancies or errors in the transcript must be reported to ATEAM immediately for correction. Transcripts that are found to be inaccurate or incomplete will be reissued at no charge to the student. 

1. FERPA Compliance: ATEAM complies with the Family Educational Rights and Privacy Act (FERPA) of 1974, which protects the privacy of student education records. Transcripts will only be released in accordance with FERPA guidelines.
2. Third-Party Requests:

• A signed release form from the student or alumni is required to authorize the release of transcripts to any third party (e.g., employers, educational institutions).
• In cases of legal disputes or subpoenas, ATEAM may release transcripts in accordance with applicable laws.
